Grace
Schools is pleased to offer its employees a wide variety of valuable benefits that
are outlined below. Grace pays for many of these benefits. Most of the benefits
outlined below begin at the time of employment. During New Employee Orientation,
each benefit will be reviewed and appropriate plan descriptions will be provided
to each new employee.
Benefit programs are reassessed periodically in light of individual and organizational
needs. As a result, benefits may change from time to time. The Human Resources
Department will issue announcements if and when changes occur.
MEDICAL
INSURANCE
Grace College provides medical coverage through SIHO (Southeastern Indiana Health Organization). For a quick overview of this benefit

Employee
pre-tax premiums are currently:
Single coverage—1.75% of gross wages per pay
Family coverage—add $70 per month (for one or more family members)
Prescriptions: Although Grace Schools provides to employees on the medical plan prescription medications at affordable co-pays, click here to find a list of $4.00 medications at any Wal-mart Pharmacy.
Preferred Provider Organization (PPO): What is a Preferred Provider Organization (PPO)? A PPO is a network of Health Service Providers who have joined together and agreed to a central "fee for service" structure in exchange for service area promotion. Grace offers the use of three PPO's:

Preferred Network (TRP): TRP covers Kosciusko County, northern Indiana and the Clarion Hospitals. There are two ways that you can see if your healthcare provider is in the Three Rivers Preferred network:

Using
Claim Forms: Claim forms are necessary only when an employee
uses an out-of-network Health Service Provider or when receiving dental
service. If you use a Health Service Provider that is in your network,
the use of a claims form will not be necessary. In network providers
will automatically follow the necessary billing process. Claim forms
may be picked up in the Business Office.
Adding or dropping a dependent? If
during new employee orientation, you chose not to add your
family to our medical plan, you will have the opportunity to
add a family member in the case of a qualifying event. Contact
the Human Resources Department for details at ext. 6080.
Adding a new spouse, adopted child
or newborn? Within 30 days of the new family addition,

DENTAL
INSURANCE
Benefit Overview: Grace
College provides dental coverage through Principal Financial. For

Employee Premiums: No additional cost
to eligible employees. This is included in the 1.75% for the medical plan coverage.

VISION
PLUS PROGRAM
The Vision Plus Program through Vision Partners Group offers a reduced rate on eye health examinations, frames, lenses, contact lenses and most other optional items available through a participating network of doctors of optometry. This is offered to all current employees regardless of enrollment in the medical plan.

HEARING CARE PROGRAM
Grace College and Theological Seminary has contracted with both the HEARUSA Network and the Hearing Center of Warsaw to bring employees discounted hearing care services.
The HEARUSA Hearing Care Network will provide the following services to Grace Employees:
- Free hearing screening exam to identify the presence of hearing loss.
- 15% discount on audio logical testing not covered by a health insurance plan.
- 15% discount on hearing aid purchases (or promotional pricing, whichever is greater).
- Statewide access to network providers: Members call 1–800–333–3389 to locate a provider in your area.
The Hearing Center, 3161 E. Center Street, Warsaw, IN, 574–267–8182, along with its statewide offices offers a 15% discount on hearing aid services to include the purchase of hearing aids and ear molds. (Hearing evaluations or physician examinations are not included in this discount program). Call 800–555–5402 to locate offices outside of Warsaw.

FLEXIBLE
BENEFITS/CAFETERIA PLAN
The Flexible Benefits or Cafeteria Plan is a program that allows employees to
receive a tax break on medical premiums, out-of-pocket medical, dental, vision
care, and dependent care expenses. Employees interested in this plan must sign
up within the first thirty days of employment. All interested employees also
must re-enroll in this plan annually or sign a waiver of participation. SIHO (Southeastern Indiana Health Organization)
is the plan administrator for Grace.
Plan Components: There are three
independent components of this plan in which employees may participate.

1.)
Tax-deferred Medical Premiums:
Employee medical premiums may be taken out of your pay before taxes thus reducing your taxable income.
2.) Medical Savings Account (MSA) (www.ezflexplan.com/siho/):
There are two benefits to setting up a Medical Savings Account. First,
employees may estimate their annual out-of-pocket medical, dental, vision
care expenses (expenses not covered by any our benefit plans) and begin setting
this money aside into a Medical Savings Account through payroll deduction
before taxes. Secondly, once this savings account is in place, claims
for paid, out-of-pocket expenses can by turned in for up to the annual
election.
Two important notes: First, be careful not to overestimate the amount you
believe you will spend in any given year. Any money left in your account
as unclaimed cannot by law be returned to you. Remaining contributions will
need to be filtered into the plan to help cover plan costs. Second, once
you select an annual amount to be set aside into this account, you will not
be allowed to change this amount unless there is a qualifying event (see
the Human Resources Generalist for details).

3.) Dependent Care Savings Account: This account works exactly like the Medical
Savings Account above, except this account allows employees to set aside
tax deferred money to cover Dependant Care expenses. For a listing of

in the Business Office.
Claims Deadline: Claims may be submitted anytime directly to SIHO by the employee via fax, mail or email (flex.info@siho.org). Reimbursements (via direct deposit/check--employee designates which one) are processed weekly on Thursdays with direct deposits made on Friday and checks sent on Friday to those not on direct deposit.

Year-end Claims: Are you scrambling in December to turn in your Medical and Dependent Care claims? Relax . . . claims may be submitted until April 30 of each year for claims incurred in the previous calendar year. If you had a balance available at the end of a plan year, you have until March 15 of the following year to spend your prior year's account balance (14 1/2 months total). This provision has been made in light of the occasional lag in claims processing by Health Service Providers.

PROGRAM
Grace has established a generous 401(k) Retirement Plan and Trust in order to provide additional retirement security for its employees. M&I Retirement Plan Services administers the 401(k) Plan.

TUITION
BENEFIT
Tuition benefits are available to both full- and part-time employees whose position is classified as part-time with benefits (PTB). In addition to gradual tuition remission in both our College and Seminary, Grace
is also part of a tuition exchange program with the Council of Christian Colleges & Universities (CCCU) and with the Council of Independent Colleges (new in the summer of 2005).

LIFE
INSURANCE
Grace provides the equivalent of one times an employee's annual salary in Life
and Accidental Death and Dismemberment Insurance. This benefit is provided to
employees at no charge. Full-time and part-time employees working at least
1,000 hours per year are enrolled in the life insurance program at the time
of employment. Eligible
employees will be enrolled in this plan during new employee orientation. Changing your Beneficiary: Print

VOLUNTARY
LIFE INSURANCE
A Voluntary Life & Accidental Death and Dismemberment Insurance policy
is available to eligible employees. Full-time and part-time w/benefits (PTB) status employees are eligible to enroll in this plan at the time
of employment. This plan allows employees to purchase up to 5 times their annual
salary in Life and Accidental Death and Dismemberment Insurance for themselves.
This plan allows up to $50,000 in Life and AD&D Insurance to be purchased
for a spouse and $5,000 of Life and AD&D Insurance to be purchased for
eligible dependents. An open enrollment period is made available each year for
those employees who initially waive this coverage. Premiums for this coverage
are paid through payroll deduction. Changing your Beneficiary: Print

LONG-TERM
DISABILITY INSURANCE
Grace provides Long-Term Disability Insurance for full-time employees in order
to supplement an employee's income during a time of qualified long-term disability.
There is a 180-day waiting period prior to the beginning of this coverage.
There is no cost for this benefit to the employee. Eligible employees will
be enrolled in this plan during new employee orientation.

SHORT-TERM
DISABILITY INSURANCE
Grace provides short-term disability coverage for full-time employees in order
to insure income protection in the event of a qualifying disability. The duration
of this coverage is 180 days. An eligible employee will be required to first
use up any accrued sick days before short-term disability coverage goes into
effect. After an eligible employee's sick days are used up, the short-term
disability coverage will be in effect for a period equal to a maximum of 180
calendar days minus the number of sick days used.
During a short-term disability period an employee will be given the following
benefits:
- For Employees
with % of base pay
- 3 or more years of employment 60% base pay
- 4 or more years of employment 80% base pay
- 5 or more years of employment 100% base pay
An
employee must provide written documentation from the attending physician
listing the nature of the disability and the resulting work restrictions,
including the length of time the employee is expected to be absent from work.
In the event that Grace Schools has reason to question the nature and/or
duration of the employee's disability, Grace may, at its own expense, require
the employee to undergo a confirmation examination by a physician of its
own choice. For the purposes of this policy, pregnancy will be considered
a qualifying disability.

EQUIPMENT
AND MATERIALS USE
Employees may use institutionally owned equipment for personal use or for personal
needs of their spouse during non-business hours. Such equipment includes typewriters
and personal computers. Paper or any other consumable materials used for personal
reasons must be purchased.

FACILITIES
USE
Robert & Frances Gordon Student Recreation Center: This facility
is available for use by employees and their spouses during staff and faculty
hours.
Library: Employees are encouraged to make use of the campus library facilities. An ID card is needed to check out books and materials and is available at no charge from the library.

SPECIAL
CAMPUS EVENTS
Institutional Events: Employees are
encouraged to attend institutional convocations, commencements, days of
prayer, chapel services, student/faculty retreats, Bible conferences, and
missionary conferences. Attendance is based upon supervisor's approval.
Sporting Events: Free admission to all sporting events, with the exception of tournaments and playoffs, is granted to all full-time employees and their families. Attendance is based upon supervisor's approval.
Artist/Lecture Series: Approximately
one program per month throughout the year is arranged for Grace
School's community enjoyment. These musical, drama, and lecture
programs are announced in advance through campus announcements.
These events do not include dramas and musical productions
by the Drama department. An employee and the employee's immediate
family are invited to attend.

CLASS
ATTENDANCE
Course work taken on the Grace Campus, whether to enhance present job skills
or for personal enrichment, must have the written approval of an immediate
supervisor.
A full-time employee may attend classes if this time off does not adversely
affect normal operations, and the standard workweek (40 hours) is maintained.
This attendance will require supervisor's approval and is subject to the eight (8) hour
maximum number of credit hours allowed any full-time staff member per semester.
All work time missed for class time must be made up within that workweek.

MEAL
PASSES
Twenty (20) meal passes are credited to full-time employees per semester during the
fall and spring semesters by way of the employee ID card for use at the Alpha
Dining Commons.

EMPLOYEE
EMERGENCY DAY HANDLING
If
the institution is closed for an "emergency day" it is considered
closed for all personnel.
When an announcement is made over the local radio station that classes have
been canceled due to a snow emergency, it is understood that all non-essential
personnel are excused from reporting to work. Service personnel may need
to report to work due to the nature of the work they perform. The supervisor
will notify service personnel.
If the institution is open, but an employee is unable to make it in to work,
or feels the need to leave work early due to inclement weather, work time
can be made up in the following manner:
Make
up the time within that week.

BENEFIT DAYS ELIGIBILITY—HOURLY EMPLOYEES

Full-Time Hourly Employees: Benefit days are granted during the first year of service to employees classified as full-time.
Part-Time w/Benefits (PTB) Hourly Employees: Benefit days are granted after three years of service to part-time employees who work over 20 hours per week for at least 48 weeks per year (1,000 hours or more annually).
BEREAVEMENT DAYS—HOURLY & SALARIED EMPLOYEES
Bereavement
leave of three work days with pay will be granted within seven (7) days following
the death of a member of the immediate family. (Immediate family is defined
as husband, wife, father, mother, son, daughter, brother, sister, father-in-law,
mother-in-law, daughter-in-law, son-in-law, step-father, step-mother, step-son,
or step-daughter, grandfather, grandmother, grandchild, brother-in-law,
sister-in-law.) Only scheduled workdays will be considered as funeral leave.
Employees will not be paid for the time they would not have normally worked.

When
a holiday falls on a Saturday, the Friday before will be the day off. When
a holiday falls on a Sunday the Monday after will be the day off.
If work or school schedules demand that employees work on a holiday, another
day off will be granted in lieu of the holiday, and the employee has the remainder
of his/her anniversary year in which to use the holiday unless arrangements
are made with the Human Resources Generalist.
When the employee is called in unexpectedly for an emergency on a holiday,
as deemed necessary by the Supervisor, that employee may choose either to be
paid "time and a half" for hours worked or have the day as the holiday
to be taken at another time during the anniversary year.
Part-Time w/Benefits
Hourly Employees:
Benefit
days are granted after three years of employment with Grace Schools to employees who hold a position with the Part-Time with Benefits status (PTB) whereby they have worked a minimum of 20 hours per week (1,000 hours or more annually)
for those three years.
Holidays: 7 paid holidays
BENEFIT DAYS—HOURLY EMPLOYEES
VACATION DAYS
Each
full-time staff employee will be granted vacation time based upon the length
of service from the anniversary date of the first day of full-time employment. After
six months of continuous employment, vacation days are available for use
with approval of supervisor.
Unused
vacation days cannot normally be carried over to subsequent years. However,
unusual circumstances beyond the employee’s control may necessitate
such a carryover, with the approval of the immediate supervisor and the Human Resources Generalist.
Vacation
time can only be taken in one-half or whole-day increments. No vacation
time will be allowed prior to it having been earned.
Since
the workload of different areas may vary at different times of the year,
vacation periods must have the individual supervisor’s approval. In
case of any conflict of desired time among employees, those with seniority
shall have preference.
Terminating
employees who have been employed one year or more are eligible to be paid
for unused vacation days accrued but unused.

SICK DAYS
The
purpose of paid sick days is to provide continued income during periods of
illness.
Sick
days may be used for the following reasons:
1. Personal
illness or medical disability which prohibits the performing of duties.
2. Absence
from work for medical/dental treatment which cannot be arranged during non-work
hours.
3. Illness
or injury of employee’s spouse, dependent children, or parents.
Sick
days is calculated and used on an hourly basis. Unused sick days may
accumulate from year to year up to a maximum of 90 days. One half of
all accumulated sick days will be paid on the last payroll check upon retirement
or termination after ten years of service. One hundred percent of all
accumulated sick days will be paid upon retirement or termination after twenty
years of service.

Vacation
Days: Each full-time administrative staff receives one
month of paid vacation defined as twenty-three
working days. The vacation time is based on the calendar year. Days
are pro-rated for employees beginning later than January 1. Unused
vacation days cannot normally be carried over to subsequent years.
Each employee is required to submit a request for vacation form PRIOR to
taking vacation days. This form can be found on the H.R. page of
the Grace web site under forms. It should be completed and approved
by the immediate supervisor and then forwarded to the Payroll Coordinator.
Personal/Sick
Days: There are no allotted personal or sick days for administrative
employees. It is understood when an administrative employee takes time
off for sickness or personal reasons, lost time can either be made up
or vacation days may be used to cover these absences. |
