  • Two Reference Forms

The Spiritual Life reference should be completed by your pastor or a spiritual mentor, and the Personal Reference form should be completed by a person who knows you well (references should not be family members). The best and quickest option is to send your references the appropriate link to the online forms (see below). They will be required to enter your first and last name on this form, so please supply them with the same name you gave on your Seminary application. If you are unable to correspond with your reference person by email, you may download the PDF Reference Forms below.
  • Church Recommendation Form

Email your church and have the proper church authority (pastor, head elder, etc.) complete the online Church Recommendation form (click here for link). The church authority will be required to enter your first name and last name on this form, so please supply this information to them. Please use the same information that you provided on your application form. If you are unable to correspond with your church by email, you may download the Church Recommendation form.
