As a result of the 1998 HEA amendments we are required to provide you with a list of required consumer disclosures. We have given you a brief description of the disclosure information and how to obtain the complete disclosure information. If you need any further assistance after reading the various publications or contacting the individual departments we ask that you contact the Office of Student Financial Aid Services.


FAMILY EDUCATION RIGHTS AND PRIVACY ACT (FERPA)
(This only applies to enrolled students)

  1. Right to and procedures for inspecting and reviewing a student’s educational records

  2. Right to and procedures for requesting amendment of a student’s education records

  3. Right to consent to disclosure of personally identifiable information contained in the student’s education records

  4. Right to file a complaint with the U.S. Department of Education

  5. Right to obtain a copy of the student records policy

FEDERAL FAMILY EDUCATION LOAN PROGRAM (FFELP)
(Loan Deferments for Performed Services)

Terms and conditions of deferments for:

  1. Service in the Peace Corps

  2. Service under the Domestic Volunteer Service Act of 1973

  3. Comparable volunteer service for tax-exempt organization of demonstrated effectiveness in the field of community service

AVAILABLE FIANANCIAL ASSISTANCE

Description of all available federal, state, local, private and institutional financial need-based and non-need-based assistance programs, and for each program a description of:

  1. Application form and procedures

  2. Student eligibility requirements

  3. Selection criteria

  4. Criteria for determining the amount of a student’s award

Rights and responsibilities of students receiving Title IV and other financial aid, including:

  1. Criteria for continued eligibility

  2. Satisfactory academic progress standards and criteria to reestablish eligibility if student fails to maintain satisfactory academic progress

  3. Method and frequency of financial aid disbursements

  4. Terms of any loans received, sample loan repayment schedules and the necessity for repaying loans

  5. General conditions and terms applicable to any employment offered as part of student’s financial aid award

  6. Exit counseling information required to be provided and to be collected from student borrowers of the FFEL or Federal Perkins Loan

Completion or graduation rates of certificate or degree-seeking, full-time undergraduates who graduated or completed their program within 150% of the normal time for graduation or completion.

  1. Statistics for the 3 most recent calendar years concerning occurrences of crime on campus, in or on noncampus buildings or property and public property

  2. Policies regarding procedures to report crimes committed on campus criminal actions or other emergencies and Grace’s response to such reports

  3. Policies concerning the security of and access to campus facilities

  4. Policies concerning campus law enforcement

Report on Athletic Program Participation Rates and Financial Support Data

  1. Number of male and female full-time undergraduates

  2. Required information regarding varsity teams that compete in intercollegiate athletics

  3. Unduplicated number of students, by gender, who participated on at least 1 varsity team as of the date of the first scheduled contest

  4. Institutional revenues derived and attributable to each sport

  5. Incurred expenses derived and attributable to each sport

  6. Total spent on athletically-related student aid awarded to men and to women

  7. Aggregate total recruiting expenses for men’s teams and for women’s teams

  8. Average annual institutional salary of non-volunteer coaches (across all sports) of all men’s teams and women’s teams on a per person and per full-time basis

  9. Average annual institutional salary of non-volunteer assistant coaches (across all sports) of all men’s teams and of all women’s teams on a per person and per full-time basis

REPORT ON ATHLETIC COMPLETION/GRADUATION RATES

  1. Number of students, by race and gender, who attended the school during the prior year

  2. Number of students, by race and gender within each sport, who attended the school during the prior year and received athletically-related student aid

  3. Completion or graduation rate, by race and gender, of certificate or degree-seeking, full-time undergraduates who graduated or completed their program within 150% of the normal time for graduation or completion
